Position Summary:

Under the leadership of the Clinical Coordinator and Hospice Director, the Hospice Chaplain is an active member of the interdisciplinary team that delivers professional hospice services and support that is consistent with the strategic vision, goals, philosophy and direction of the hospice department and CRHS. The Hospice Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Crisp Regional Hospice, either directly or through coordination of care with other spiritual counselors.

Basic Qualifications:

• Education :

Requires a degree from an accredited seminary or school of theology (BD, MDiv or equivalent theological degree). CPE units preferred.

• Experience:

Experience with families/caregivers and patients with death/dying issues. Ability to work as member of interdisciplinary team. Comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs, and values. Ability to network with community clergy and congregations. Knowledge and commitment to hospice philosophy of care. Experience with medical documentation.

• Licensure, Registrations & Certifications:

Requires and maintains a current valid driver’s license and proof of minimum automobile insurance. A Motor Vehicle Report is required upon hire and reviewed annually.

Essential Job Responsibilities:

• Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
• Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison and support to community chaplains and spiritual counselors.
• Provides consultation, education, and support to the interdisciplinary group on spiritual care.
• Provides for funeral or memorial services for patients as requested.
• Provides support services to the Inpatient Departments and Emergency Department as needed.
• Accepts and performs other related duties and responsibilities as required.
